Providing a Memorial Membership is a wonderful way to honor your deceased loved ones.   Memorial Membership fee is $150.00.

Membership includes your choice of either a membership certificate or a bronze grave marker.  Your loved one's name and farming ancestor will be listed on our website.  Please select your choice on the Memorial Membership application.

INSTRUCTIONS

Download the Memorial Membership Application and fill it out.  Please use black ink.  Scan your completed and signed applications and proofs, for each applicant, then email them  to: NSDOAF@gmail.com.   

 

Next, proceed to pay the Memorial Membership Fee by clicking on the Memorial Membership graphic below.  We will contact you if we have any questions regarding your application.

Online Memorial Membership fee $150.00 per Memorial Membership application, plus an online transaction fee of 3%  (total: 154.50).
